Our Practice

Which hospital(s) are you affiliated with?

We have no official affiliation with any local psychiatric hospitals. However, we have relationships with several providers that care for patients when admitted to inpatient facilities. 

Do you see patients on weekends or evenings? 

Our office hours are 9:00 am to 4:30 pm, Monday - Friday. 

Do you make same-day appointments or accept walk-ins?

We do not accept walk-ins nor schedule same-day appointments. 

Visits

How far in advance should I schedule my appointment?

Staff will schedule appointments at the discretion of the psychiatrist and availability. 

What should I expect during my visit?

Courtesy and compassionate care. 

Office Policies

Is there a cancellation fee?

You must cancel at least 24 hours before your scheduled visit, or you will be charged a $25 no-show fee.

What if I am running late for an appointment?

We allow a fifteen (15) minute grace period. If you arrive more than 15 minutes late for your appointment, it will be at the physician's discretion whether you will still be seen or have to reschedule.

Can I bring my children with me to my appointment?

Our waiting area is small and permits seating for only a few persons. 

How do I request my medical records?

You may request your medical record via our https://patientonlineportal.com or by contacting the office directly. 

Do you complete FMLA, Disability or Workers Compensation forms?

No. Our practice will not complete FMLA, Disability, or Workers Comp forms unless they are deemed medically necessary. 

Medication

How do I request a refill?

Effective May 1, 2023, refills will not be provided to those patients with an outstanding balance or who have not been seen by your physician. After you have contacted your pharmacy directly, all prescription refills can be submitted electronically to your pharmacy, or you may call in a request to your physician's nurse. Please allow 24-48 hours processing time for all prescription requests (pending approval). 

Insurance & Billing

Do you accept my insurance?

We accept many different insurance carriers. Please visit our Billing & Insurance page to view the list.

You are responsible for ensuring that we have valid insurance information on file for you. You must present your insurance card at check-in for each visit. We may ask you to reschedule your appointment or pay cash for services provided if you cannot present your insurance card upon check-in.

What if I do not have medical insurance?

If you do not have health insurance or cannot produce the appropriate documentation to verify coverage, you will be a "self-pay" patient.

Payment is required upon check-in. Charges will be determined based on customary coding and billing practices and will reflect the nature of your visit and the services received. You will receive an additional bill from a separate lab facility for any lab work that is performed.

What forms of payment do you accept?

Payment of the patient responsibility portion of your visit is due at the time of service, including all applicable co-payments. We accept cash, Visa, MasterCard, Discover, and American Express for your convenience.

What if I need a letter or form completed by my doctor? 

Letters and forms can be completed for established patients only. Letters may require a fee of up to $50, and multiple-page forms will require a fee of up to $150 per hour for completion.
